May it be hereby known that the Client  (Licensee and Purchaser) :

has entered into contracted agreement with the Licensor and Vendor :

and with the Author of the work(s) hereby itemized:

Whereby it is mutually agreed that:

1.)                Vendor licenses mechanical photo-reproduction by the Client of the score(s) itemized below:

2.)                Vendor licenses a maximum of 50 copies each of the score(s), with additional copies requiring purchase and issue of a new license

3.)                All copies of licensed score(s) are intended solely for use by the Client institution (as entered above), and licensed exclusively for study, rehearsal and/or performance.

4.)             Client agrees that no copies of licensed score(s) be shared with any other entity than as specified above, unless written permission is petitioned by the Client and granted by the Vendor.

Above and beyond the terms of the licensing contract stipulated herein, Client agrees to abide by all rules and regulations protecting registered copyrighted materials [see copyright notice below, which also appears in the page(s) of Notes that accompany the itemized score(s)] , i.e. :

Instructions for Use

1.)        Download PDF file for Contract.    CLICK  HERE  for download.
2.)        Print Contract Agreement (2 pages).
3.)        Fill out form (with exception of identification number blanks).
4.)        Mail completed form and payment to the address above.
5.)        A countersigned photocopy of the Contract Agreement, with identification numbers
            added, will be included with the music shipment, for reference and filing by the Licensee.
